Despite decades of scientific advances and innovation, cardiovascular disease persists as one of the greatest public health challenges of the modern era. High LDL-C (low-density lipoprotein-cholesterol) is a leading modifiable risk factor for heart attack and stroke. Despite updated clinical guidelines aimed at managing elevated LDL-C levels, many patients remain at risk of a CV event fall short of reaching the recommended thresholds.

This session will explore successful real-world collaboration across the healthcare system to augment standardized pathways for patient care using an innovative approach. These evidence-based initiatives aim to enhance patient outcomes, deliver cost efficiencies, and ultimately promote population health.
